Summit Award Nomination Form

Thank you for taking the time to nominate an outstanding individual for a Summit Award! This is your chance to recognize the caregivers, advocates, patients, and healthcare professionals who make a lasting impact in the community.

The Summit takes place on September 13-14, where we’ll come together to connect, learn, and honor those who inspire us. Award winners will be announced at the September 13th dinner reception and will be notified by the end of August. Winners will also receive complimentary registration for the full summit, though travel and accommodation costs are not covered.

Best Policy Advocate – Recognizing someone who champions policy changes and raises awareness to improve the lives of patients.

Patient of the Year – Honoring an individual living with lupus, Crohn’s, or another condition who inspires others through their strength and advocacy.

Best Provider – Celebrating a healthcare professional who delivers outstanding patient care and support.

Best Patient Support Team – Recognizing a group that provides exceptional assistance, resources, and encouragement to patients.
